Post Placement Aftercare

At Martin De Porres Youth & Family Services (MDP), we know that when our youth leave care and no longer have the strict structure of adults caring, supporting, encouraging, and monitoring their progress 24 hours a day, it can be a vulnerable time for them. They often return to those same communities and family circumstances that played a significant role in why they were placed in care.

MDP provides aftercare services for youth who are transitioning out of our juvenile justice and foster care programs.  Aftercare provides essential services for youth and families as the youth transitions back into the family’s home and the community. The Case planner and aftercare team develop an aftercare plan with the youth and family including a complete trauma assessment, assessment of personal and external risks, individual and family needs, and the ability of the youth and family to benefit from the provided services. Aftercare services include but are not limited to medical, mental health, recreational, educational, vocational and employment areas. In addition, youth in aftercare will continue to participate in the agency’s special youth development programs including Motivational Mondays, the MDP Mentoring for Success Program, and any appropriate recreational activities during the week. Aftercare services are provided in the family’s home, service provider partner sites in the community, and community events or recreation sites.
