Health Home and Care Coordination

What is a Health Home for Children? 

 

A Children’s Health Home is an innovative model for delivering critical healthcare and other services to support a vulnerable child’s complete wellbeing. Children with complex medical and other treatment needs often do not receive integrated and coordinated care, as they are served by multiple providers without a mechanism for consistent communication, information sharing and planning amongst them. Their parents and care givers may often not be equipped to fulfill this role or may not have the access to services that can support them in that effort. In a Health Home, a network of organizations and providers work together, and each child and family participating is supported by a Care Manager who coordinates physical, behavioral and other social support services from across the network as well as among, when necessary, providers outside the network. The Health Home Medicaid service model allows for a child’s care providers to communicate and share records so that services are not duplicated or neglected, therefore, the focus remains on strengthening families to prevent crisis which paves the way for long term success and wellbeing. Martin de Porres Youth and Family Services provides care management services as a member of the Collaborative for Children and Families Health Home, which is a consortium of 47 agencies throughout New York City and the surrounding suburbs.

What are Care Coordination Services?

Care Coordination services are available to Medicaid-eligible children in New York State with one or more qualifying conditions. All children within a family who meet the required criteria are eligible, and children can qualify even if they are enrolled in other services, such as mental health, preventative, or foster care. To qualify, a child must be eligible for Medicaid and have one single qualifying condition or two or more chronic medical conditions, listed below.

  • HIV
  • Serious Emotional Disturbance or Complex Trauma
  • Diabetes
  • Asthma
  • Obesity and Metabolic Diseases
  • Cardiovascular and Respiratory Diseases
  • Attention Deficit / Hyperactivity Disorder
  • Depression
  • Most Mental Health, Personality, and Psychiatric Disorders and Diseases
  • Eating Disorders
  • Alcohol and Substance Abuse Disorder
  • Recurring Health Issues
